Care Central VNA delivers care to communities and individuals through the following programs: Home Health Services provide a broad array of home health care, in coordination with individual patient care needs and as directed by the attending physician. This includes assistance from registered nurses, home health aides, therapists, dietician and social workers as needed. The Hospice Program provides end-of-life care for those living with terminal illness, emphasizing holistic comfort care as delivered by an interdisciplinary team of professionals, and honoring patient choice. The Adult Day Services program meets the physical, functional and social needs of elderly and disabled adults at4 Adult Day Health Care centers in Gardner, Athol, Fitchburg and Greenfield. Assisted Living Programs provide care for elders and the disabled which allows them to remain in their own homes where supportive services are provided (home health aides, nursing, social work). This program is a part of the Group Adult Foster Care Program overseen by the MA Division of Medical Assistance. Care Central VNA has agreements with 23 apartment complexes in 9 towns in our service area. Community Health and Education Programs are offered within the community through flu clinics, blood pressure screening, specific educational topics, and support groups. Our Maternal Child Health Program offers a wide array of services specifically designed to meet the needs of pregnant women, infants, children and their families. Private duty services provides another option for an individual to comfortably be cared for in their own home with just a little extra help from qualified homecare professionals. It offers a private pay service package specifically tailored to a client's needs. Services may include nursing visits, medication management, personal care , homemaking, companionship, transportation for errands and medical appointments, as well as respite for caregivers. Community Services Center in Fitchburg offers programs and services to address needs associated with extreme poverty, addiction, homelessness, and other at-risk behaviors. Programs include food pantry, housing case management, treatment referrals, transportation assistance and substance abuse recovery readiness counseling.

Care Central VNA was founded in 1906, by a group of local nurses and physicians. Since its inception, the agency has grown dramatically while continuing to uphold the ideals and goals of our founders: providing affordable, high quality health and social services to persons who need them. Today, thousands of visits are provided on a yearly basis, as we seek to remain true to the belief that people deserve skilled, compassionate health services in the comfort and security of their own homes.
